PREVENTABLE MEDICAL ERROR
Preventable medical error — including diagnostic error, medication harm, hospital-acquired infections and surgical mistakes — is estimated to be among the leading causes of death in several developed countries. A Johns Hopkins study (Makary & Daniel, 2016) estimated that medical errors cause roughly 250,000 deaths per year in the United States, ranking it the third leading cause of death.
The World Health Organization (WHO) Patient Safety programme reports that 1 in 10 patients is harmed while receiving hospital care in high-income countries, and that most of this harm is preventable. Figures vary widely because countries define, count and report medical harm differently — many do not publish national totals at all.
National estimates below are drawn from the most authoritative public source available per country; where a country does not publish a figure it is shown as "Data not publicly available".

MEDICATION & CROSS-MEDICATION ERRORS
The WHO "Medication Without Harm" global patient safety challenge identifies medication errors — including cross-medication errors from look-alike/sound-alike drug names, wrong dosing, and prescribing errors — as a leading, largely preventable cause of patient harm worldwide.
